How to Refer a Patient

Enrollment for ULTRA closed July 1, 2022. 

Remember, all patients must enroll in ULTRA BEFORE having the Acessa or myomectomy procedure.

1. Confirm patient is planning Acessa or myomectomy.
2. Inform patient about ULTRA:

• Refer to this website: fibroids.ucsf.edu
• Brochure can be given to patient (Request brochures at [email protected])

3. Confirm patient gives permission for contact by UCSF ULTRA study*
4. Email [email protected] with: 

• Patient Name
• Email
• Phone Number
• Procedure type
• Scheduled date of procedure

*Use the Permission to Contact form to obtain patient consent.
